Responsibilities

SUMMARY - The Nurse Navigator is a professional registered nurse with disease-site clinical knowledge who is assigned to a population of patients. The Nurse Navigator provides individualized and

comprehensive care to recognize and removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health. to care through, coordinate diagnostic and treatment efforts and ensure prompt access to care. The Nurse Navigator provides education and resources to facilitate informed decision making and timely access to quality health throughout all phases of the continuum of care. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to effectively coordinate patient care,

ensure continuity of care and create a comfortable and exceptional patient experience.

Qualifications

MINIMUM

ADN (Associate Degree Nursing)

Three years of experience of specialty specific nursing in a hospital, clinic, or community setting.

Skills to include but not limited to knowledge of self-care strategies and resources, critical-thinking skills, ability to work autonomously, strong verbal and written communication skills, ability to prioritize, ability to build internal and external collaborative partnerships.

(1) Demonstrated knowledge of medical terminology.

(2) Knowledge and understanding ofprocesses and systems related to registration and patient eligibility systems.

(3) Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service.

(4) Knowledge of the principles andprocesses for providing effective customer service, including customer needs assessment, meeting quality standards for delivered services and evaluation of customer satisfaction.

(5) Excellent organizational skills and the ability to handle multiple priorities/tasks simultaneously in a fast paced environment.

(6) Basic computer literacy and proficiencyin Microsoft Windows, including experience using multiple software/computer applications simultaneously.

(7) Basic proficiency with MS Office(Outlook, Word, Excel, PowerPoint).

(8) Ability to effectively capture informationand accurately enter the data into software applications.

(9) Proficiency with patient registrationcomputer applications.

Must have current or compact RN licensure in the state of Arizona

American Heart Association BLS-CPR

$33.60 - $48.73 /hour
