JOB SUMMARY Coordinates, negotiates, communicates, and manages the care of assigned patient population along a continuum to facilitate achievement of optimal patient outcomes within appropriate LOS and resource utilization parameters. Education:Graduate of a school of nursing. BSN or MSN preferred. Experience:Two (2) to Three (3) years recent clinical nursing experience in an acute care setting. License and Certification:Current unrestricted License in good standing to practice as a Registered Nurse in the State of Connecticut. Current Basic Life Support (BLS) certification. Ability to obtain and maintain department specific certifications. Certified Nurse Care Manager preferred. We take great care of careers.
