Join our team at Preferred Family Healthcare as a Care Coordinator!As a Care Coordinator, you'll review initial assessments, coordinate outpatient services, maintain communication with referral sources, assist in transitional planning, and provide crisis intervention when necessary. Key Responsibilities:

  • Comprehensive Assessment: Review initial assessments and identify issues associated with employment, education, healthcare, social connectedness, and criminal justice, laying the groundwork for holistic treatment planning.

  • Outpatient Service Coordination: Coordinate outpatient services for mental health and/or substance use disorder treatment, ensuring seamless access to essential care and support.

  • Effective Communication: Maintain regular communication with referral sources and primary care providers regarding progress, transition planning, and pertinent clinical issues, fostering collaboration and continuity of care.

  • Correspondence Management: Request correspondence from past treatment and primary care providers, facilitating comprehensive understanding of clients' medical and treatment history.

  • Staffing Participation: Participate in staffing to assure continuity of care, contributing insights and updates to inform collaborative decision-making and care planning.

  • Treatment Scheduling and Transportation: Assist in scheduling treatment and arranging transportation, removing logistical barriers to accessing essential services and support.

  • Individualized Treatment Planning: Develop and evolve treatment plans in an individualized manner, tailoring interventions to address the unique needs and goals of each client.

  • Professional Representation: Represent the agency in a professional manner, fostering positive relationships with clients, families, and community partners.

  • Referral and Linkage: Ensure referral for medical issues of clients and link families with services in the community, facilitating access to comprehensive support networks.

  • Crisis Intervention: Provide crisis intervention as necessary and adhere to weekly direct service expectations, ensuring prompt and effective response to emergent client needs.

Education and/or Experience Qualifications:

  • Graduate from an accredited college or university with a Bachelor’s Degree in one of the human service fields (social work, psychology, human development and family life, criminal justice, counseling), nursing, or education.

  • Strong interpersonal skills: Ability to establish rapport with clients, families, and multidisciplinary team members to facilitate effective communication and collaboration.

  • Detail-oriented: Demonstrated ability to maintain accurate records, document client interactions, and ensure compliance with regulatory standards.

  • Crisis intervention skills: Proficiency in de-escalating crisis situations and providing immediate support to individuals experiencing mental health or substance use issues.

  • Flexibility and adaptability: Capacity to work in a dynamic environment, handle multiple tasks simultaneously, and respond to changing priorities while maintaining a client-centered focus.
