Great work you’ll do here :
The ILS Worker provides clients with the training and resources needed to develop the skills necessary for independent living. The goal is to maximize client independence in the community.
Communicate monthly or more often as needed with client’s assigned case managers; Follow case chronology procedure
Report to supervisor all important client issues; follow case chronology procedure
Recognize the need for IPP goal changes and communicate this need to ILS Supervisor
Initial development of Individual Program Plan (at SOC)
Periodic Individual Program Plan Reviews (bi-annually or more often as needed)
Completion of the weekly client time cards and weekly visit time slips
Knowledge and ability to complete Client Transfer and Leave of Absence forms; Adult Protection reports; update access list weekly
Respectful to schedule on time for appointments/meetings within 5 minutes
Be prepared for monthly supervisory meetings and periodic Individual Program Plan reviews (follow procedure)
Communicate appropriately with the ILS Supervisor (follow Worker Expectation)
Respond to pages and voicemail in a timely manner (follow Worker Expectations)
Follow through on requests from the case managers or supervisors within 24 hours
Open to assisting with additional responsibility when needed
Follow guidelines to prevent client cancellations
Maintain an ILS folder in the clients’ home
Basic knowledge of diagnosis of assigned clients
Wiliness to maintain professional boundaries with all client’s assigned and establish a therapeutic relationship
Ability to work in a variety of settings
Drive clients following established guidelines (with approval and verification of Driver liencsure)
Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

Promotes the agency through positive representation and communication of its services
Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
Creative thinker who embraces thought leadership
High School Diploma / GED
Experience caring for clients/patients in need and assisting with daily activities of life
Driver license required if assignment requires driving
Current CPR certification
